The focus of your role will be to act as Responsible Pharmacist, provide support to the rest of the pharmacy team and support the development of the business. This means that in undertaking this role the post holder is expected at all times to behave in a way that demonstrates commitment to the delivery of respectful and compassionate care across all sites.VISA SPONSORSHIP DOES NOT APPLY TO THIS ROLEMain duties of the jobThe Senior Bank Pharmacist assists the section lead pharmacist in managing and coordinating their respective clinical section. This includes the responsibility for overseeing the day-to-day provision of ward pharmacy services, including allocation of staff and provision of cover in the event of sickness. The responsibilities will include:1.Assisting in the planning and delivery of a clinical pharmacy service.2.To ensure development of patient group directions and other relevant policies and guidelines.3.Delivery of a high quality, specialist ward based pharmacy service including medicines reconciliation, drug supply, review of highly complex medication regimens, interaction monitoring, adverse drug reaction monitoring and discharge planning.4.Promotion of high quality cost-effective prescribing.5.Ensuring compliance with Trusts policies including medicines policy, formulary and microbiology policies.6.Making evidence based decisions, in partnership with patients and other professionals within the specialist area.7.Making clinical decisions where limited information is available.8.Development and review of Patient Group Directions.RESPONSIBILITY FOR PATIENTS1.To assist in identifying and managing changes necessary to maintain a high quality clinical pharmacy service responsive to patients needs and to consider the implications of changes for other areas/personnel2.To ensure timely supply of medicines for discharge to reduce any delays / bed blocking3.To oversee the clinical safety of dispensing functions for off-site units and external parties4.To deal effectively with situations where medication errors have occurred, including liaising directly with the patients and staff concernedRESPONSIBILITY FOR LEADING AND MANAGINGThe post-holder will be responsible for maintaining their professional responsibilities and for identifying and addressing their own developmental needs. They are also expected to contribute to the learning and development of other members of the health-care team.
