Job Description Under the direction of the Director of Nursing Services, Clinic Administrator and/or Nurse Manager, the Registered Nurse assumes the responsibility for application of the nursing and clinical process. The Registered Nurse organizes the provision of nursing care in a Federally Qualified Health Center (FQHC) serving families in the San Fernando and Santa Clarita Valleys. The Registered Nurse is responsible for direct observation and interpretation of data to formulate a nursing diagnosis. The Registered Nurse will assist the multi-disciplinary health care team in the planning, coordination, monitoring, and evaluation of the development and implementation of care plans in accordance with Federal, State, Agency and various programmatic regulatory requirements. Reports to: Director of Nursing Services, Clinic Administrator, Registered Nurse Supervisor and/or Nurse Manager Qualifications: 1. Must be a graduate of an accredited school of nursing with a valid and current California Registered Nurse license to practice., 2. One year of clinical experience as a registered nurse, preferably in an outpatient setting., 3. Must demonstrate through written and/or oral testing, the knowledge and skills necessary to provide care appropriate to the age of the patients served and has knowledge of the principles of growth and development over the life span. Able to identify and categorize each patient’s age specific grouping of needs, such as those for infant, adolescent, adult or geriatric patients., 4. Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures., 5. Math skills sufficient to calculate medication dosage, read measurements to prepare medications and complete vital signs., 6. Nursing leadership skills., 7. Computer skills in Microsoft Office programs (Word, Excel, etc.), electronic health record systems, and database systems., 8. Effective verbal and written communication skills to communicate clearly and effectively with patients and others., 9. Effective work organizational skills., 10. Ability to work effectively as a team player., 11. Ability to be flexible and work in a changing environment which may include Saturday’s and weeknights., 12. Sensitivity to the different cultures and populations., 13. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles., 14. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision. Self-starter, reliable and dependable., 15. Demonstrated proficiency with the electronic health record database within three months of attending training session(s)., 16. Fluency in English (speaks, read and write)., 17. Fluency in Spanish (speak, translate, read and write) is preferred., 18. Ability to solve problems and make routine recommendations., 19. Ability to maintain absolute confidentiality about health care and other patient/client information., 20. Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job). RN Triage responsibilities: 1. Organizes and renders nursing triage services, both by telephone and face to face, for adult, pediatric and Women’s Health patients requiring clinical services who do not have scheduled appointments., 2. Ascertains the type and severity of the illness of the patient by obtaining a history of present illness and asking clarifying questions based on patient complaints., 3. Makes a nursing assessment over the phone, using best-practice guidelines and clinical judgment skills., 4. Formulates a care plan, in collaboration with the client and Provider if need be, which ensures that high quality patient care, health education and follow-up disposition as needed., 5. Discusses care plan with patient and family and ensures understanding of the plan including any follow-up needed., 6. Uses appropriate nursing judgment to refer those patients requiring medical intervention to the services of an appropriate medical provider for same day visit or depending on acuity within a reasonable timeframe based on patient symptoms., 7. Uses appropriate nursing judgment to refer those patients requiring emergency medical intervention to a local hospital emergency room and/or determine appropriateness of calling 911., 8. Acts as the client’s advocate, as circumstances require, by initiating action to improve health care or to change decisions or activities which are against the interests or wishes of the client, and by giving the client the opportunity to make informed decisions about health care before it is provided., 9. Using appropriate nursing judgment, determines those patients whose illnesses require nursing intervention only, and, in accordance with Federal, State, Agency and various programmatic regulatory requirements, recommends the appropriate nursing intervention and instructions to the patient., 10. Documents the above actions correctly into the patient’s electronic medical record and appropriately for each patient triaged.
