Job Description Title: Medical Coordinator Location: Chicago, IL Schedule: Tuesday to Saturday (4:30 pm - 12:00 am) Wednesday to Sunday (8:00 am - 4:30 pm) Summary: The Medical Coordinator plays an essential role in ensuring that every child in our care receives the highest standard of medical support. This position ensures all medical requirements are met in full alignment with the Office of Refugee Resettlement (ORR) and Department of Children & Family Services (DCFS) policies and standards. As a Medical Coordinator with Heartland Human Care Services (HHCS), you’ll support medical providers and advocate for the health and well-being of Unaccompanied Children, youth who have arrived in the United States without parents or documentation. You’ll oversee key responsibilities such as appointment scheduling, maintaining medical inventory, managing records, and facilitating clear communication across care teams. Our Unaccompanied Children’s Programs are diverse and multilingual, representing more than a dozen languages and cultures. BCBS PPO, EPO, and HSA Plans, • Dental insurance., • Vision insurance., • 401(k) 3% Employer Contribution., • Paid maternity leave., • Paid paternity leave., • Commuter benefits., • Professional Development Assistance., • Disability insurance, • FSA Spending Account, • Life Insurance, • Employee Assistance Program Duties & Responsibilities • Functions as the primary point person for coordination of participants’ medical care from intake to reunification., • Arranges and coordinates appropriate health services, including urgent, emergent, and preventative health services, for children in care., • Maintains complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health-care appointments, and coordinating transportation to health-care appointments., • Ensures qualified interpretation and translation services are in place and serves as a liaison for community health-care providers., • Organizes laboratory test requests for all participants in care., • Facilitates referrals to all medical specialties including dental and procedures/surgeries, which range from minor to major/complicated, for all participants while in HHCS’s care., • Obtains durable medical equipment as needed., • Orders required tests to established laboratory and imaging provider as per ORR protocol., • Serves as a liaison between HHCS medical providers and other healthcare providers outside of HHCS., • Establishes professional relationships with new medical providers outside HHCS for specific medical needs of the participants., • Facilitates public health educational workshops on mental health, sexual health, substance use etc. under the guidance of the medical directors and medical team., • Educates participants on social determinants of health, teaching participants how to advocate for their own health., • Justifies recommended laboratory requests/procedures/referrals/ER and hospital admission/Ambulance use for insurance authorization and enrolls newborns into Medicaid as needed., • Prepares all relevant documents (placement and medical authorization), insurance cards, referral notes, results, and authorizations needed for offsite medical/dental appointments., • Ensures all medical/clinical notes from internal and external appointments/procedures are encoded in the UC portal., • Uploads all laboratory/imaging results to the UC portal and informs HHCS medical providers of results that require interventions or treatments., • Receives and answers emails, telephone calls related to participants’ medical care., • Coordinates prescriptions and carries out orders from HHCS medical providers., • Provides support to clinical team to implement pharmaceutical therapy and documents progress of participants in short-term case management treatment plan., • Requests insurance pre-authorization of medical services, including tracking and resolving payment discrepancies. Troubleshoots issues with insurance and funder in regard to treatment and medication authorization discrepancies., • Maintains adequate medical supplies for the facility through providers such as but not limited to Henry Schein, Walgreens and Walmart., • Performs data entry and file management to maintain compliance with ORR., • Maintains complete, confidential medical records for., • Tracks potentially infectious reportable diseases as per ORR policy., • Writes quarterly reports and conducts medical audits., • Completes internal communication forms for medical incidents as needed., • Ensures all medical documentation adheres to ORR policies and standards., • Determines “medical clearances” of participants prior to reunification., • Leads presentations on medication monitoring to program staff periodically., • Updates internal case management database with participant information such as allergies, prescribed diet, or medications prescribed., • Masters the use of various platforms and software apps which include but are not limited to: UC Portal, Salesforce (CMS), Quest Diagnostics, TridentCare, PCU, Microsoft Office and AT&T Office @Hand., • Provides transportation for participants as necessary., • Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care AND, • Ability to fluently speak in a second language is preferred but not required., • Knowledge and familiarity in medical terminology/CPT/ICD codes for billing purposes., • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner., • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability., • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Human Care Services., • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form., • Ability to work independently and remain self-motivated with minimal direct supervision. This is a completely on-site work arrangement, with required meetings on a regular basis (weekly, monthly, etc.). Work schedules may vary and can include weekends or later hours. Work Environment: • The noise level in the work environment is usually moderate., • Each program site operates on a 24/7 basis, 365 days per year. To meet both the needs of our participants and contractual obligations, employees are required to be flexible in regard to scheduling and work location., • Program sites are located throughout the Chicago area, including the North and South sides of Chicago. Employees may be required to report to any one of our locations on occasion., • While performing the duties of this job, the employee is regularly required to talk and hear., • The employee is regularly required to sit, stand and walk., • The employee must be able to stoop, kneel and/or crawl., • The employee is regularly required to use hands to key, handle, or feel and reach with hands and arms., • The employee must occasionally lift and/or move up to 25 pounds., • The employee must have unrestricted ability to provide physical restraint., • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
