We value our families and community: • Paid time off, • Hardship Program-program tosupport staffduring challenging times, • Patriot Cares-nonprofit forcommunityoutreach How we support you professionally: • This position is paid at a competitive salary, • Mileage reimbursement for any company travel, • Career growth and potential for advancement, • Medical/Dental/Vision Insurance/Supplemental insurance(beginsimmediatelyupon hire), • Company 401K with4%company matching The responsibilities of our Branch Manager include: Clinical Leadership & Patient Care • Oversee all skilled nursing services delivered under the Home Health Benefit, • Ensure care is provided in accordance with physician orders, Plans of Care, and patient goals, • Monitor, review OASIS assessments for accuracy, timeliness, and compliance, • Ensure infection prevention, safety, and evidence-based clinical practice standards are met, • Monitorclinical outcomes, rehospitalizations, adverse events, and patient satisfaction, • Serve as the agencys primary clinical adviser to staff, patients and business associates Compliance/Quality • Ensures full compliance with Medicare Conditions of Participation (CoPs), CHAP accreditation standards, and state regulations, • Maintainconstant survey readiness for CMS validation, state, and CHAP surveys, • Participatein regulatory and accreditation surveys and corrective action plans, • Ensure compliance with Face-to-Face encounter requirements, • Ensure clinical documentation supports medical necessity and payor billing requirements, • Support audits, ADRs, medical records requests and payer reviews, • Participatesin the QAPI program in alignment with corporate goals,CMSand CHAP standards, • Analyze quality data, trends, and risk indicators, • Implement performance improvement initiatives, • MonitorStar Ratings, HHCAHPS, and HHVBP outcomes Staff Leadership & Development • Recruit, hire, supervise, train andretainqualifiednursingstaff, • Ensure staff competency, licensure, and education compliance, • Conduct performance evaluations and disciplinary actions, • Promote a culture of accountability and patient-centered care Operational & Interdisciplinary Collaboration • Collaborate with physicians, therapists, intake, QA, and leadership, • Support referral acceptance decisions, • Assistwith staffing models and census management, • Participatein leadership meetings and strategic planning Qualifications: • Graduate from an accredited School of Nursing, • Licensed in the state as a Registered Nurse, • 35 years Medicare-certified home health experience, • Knowledge of Medicare and Medicaid guidelines, • Criminal background check, • Must have a current CPR (BLS) certification, • CHAP survey experience preferred Job Conditions: • Valid driver's licenserequired, • Primarily a desk job which involves sitting, standing, stooping and walking, • Ability to access patients' homes, which may not be wheelchair accessible., • Sufficient hearing, eyesight, and physical dexterity for patient assessments and care, • Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients Patriot at Home is an equal opportunity employer
