Job DescriptionDescription: • Supports the Mission, Values and Vision of Lydia Care Center and the facility., • Is knowledgeable of Consumer rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all consumers in a safe, secure environment., • Supports, cooperates with, and implements specific procedures and programs as outlined within the facility handbook, on-going in-service training, etc., • Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment. o Follows up as appropriate with supervisor, co-workers or consumers regarding reported complaints, problems and concerns. o Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned. • Nursing staff must arrive on time daily for your designated shift: 7:00am-7:30pm or 7:00pm – 7:30am., • Upon arriving for designated shift, complete an initial round of assigned unit and verbal report with nurse being relieved., • A mandatory narcotic count must be completed by both Licensed Nurses., • The Charge Nurse is responsible for all laboratory results and the prompt relaying of any abnormal values to the appropriate Medical or Psychiatric M.D. (a) No labs are to be placed on the clipboard and/or any other location without addressing them if there is a need to leave them for the next shift nurse for follow up; (b) If you are not able to reach any M.D. by phone after two (2) hours, you must notify the Director of Nursing (DON), or the specified designee, immediately for an intervention. • The Charge Nurse will be responsible for transcribing all orders to the MAR, completing lab requisitions, filling out appointment notification form, and endorsing on the 24-hour report the need for appointment, etc. after Physicians’ in-house rounds. All orders must be completed within a two (2) hour window. • The NOC shift Charge Nurses are responsible for auditing the POS’ for the Monthly Change over each month on the 6th . You will be held accountable for any orders not transcribed properly to the new MAR’s. • All Charge Nurses will be assigned an audit assignment, and will be responsible for completing and turning in completed audits to the DON. (a) All nurses are responsible for addressing any abnormal findings immediately, and also the notifying the DON. • All Charge Nurses are responsible for the cleanliness of their assigned medication room, carts, cabinets, etc., • All nurses are responsible for completing all documentation, MAR’s/TAR’s, etc. by the end of their designated shift. If for any reason you have not completed your assigned duties, you must notify the DON, or her designee, of the reason as to clock-out/on time, excluding emergencies. • All Charge Nurses are to inform the DON, or specified designee, immediately if any Consumer is without medication, including narcotics. Certificates, Licenses, Registrations, or Educational Requirement: • Valid LPN/RN license issued by Illinois Department of Professional Regulation., • Valid CPR certification. Qualifications, Skills, and Abilities: • Excellent reading, writing, grammar using the English language., • Ability to make independent and good judgement decisions within proper policy and procedures., • Ability to manage multiple priorities/task., • Exhibit strong judgment and decision-making skills., • Computer literate., • Uses tactful, appropriate communications in sensitive and emotional situations., • Auditory and visual skills., • Excellent reading, writing, grammar using the English language., • Ability to make independent and good judgement decisions within proper policy and procedures., • Ability to manage multiple priorities/task., • Exhibit strong judgment and decision-making skills., • Computer literate., • Uses tactful, appropriate communications in sensitive and emotional situations., • Auditory and visual skills., • Must be able to work within and promote a team atmosphere. Physical Demands: • Ability to bend, stoops, sit, stand, reach, and lift items weighing 50 pound or less., • Satisfactory completion of post-offer, pre-employment physical as required., • Satisfactory completion or proof of current TB vaccination., • Able to work and/or adjust working hours to meet the demands of the department and/or facility events . Other Duties: • Performs other duties as required or assigned which are reasonable within the scope of Lydia Care Center and/or any subsidiary entities., • Assist the immediate supervisor with various administrative tasks to support department operations as directed; may also be assigned special projects., • Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.
