These positions will work to reduce disparities and improve equity in purchase of services, identify and resolve barriers to accessing services and supports; enhance the relationship between individual and family and RCEB. The overriding goal is to empower individuals and families who speak another language and/or are from communities of color and currently have low to no purchase of service, identify and to partner with RCEB and other community agencies to overcome barriers to those services. The position works in collaboration with RCEB's Cultural Diversity Specialist and other Departments within RCEB to identify innovative approaches to service delivery. These positions are the result of new funding made available in the 2021/2022 California state budget. NATURE OF WORK: Under the general direction of the Supervisor, the Specialized Case Managers in these positions will provide enhanced case management and advocacy for individuals and families on caseloads comprised of individuals currently with no purchase of service authorizations at all or low purchase of service authorizations. Using person-centered planning to address the complex needs of those individuals and families who speak another language and/or are from communities of color. The Specialized Case Manager will demonstrate culturally competent skills including valuing diversity, cultural self-awareness and knowledge of other cultures. The Specialized Case Manager will assess for cultural and linguistic barriers and identify the unmet needs of the individual and family. Upon that determination, the Case Manager will empower individuals to partner with Regional Center and other community agencies, and assist in identification of new resources that are culturally relevant to our community. This position will also be responsible for tracking and monitoring of trends which may include creating reports and calculating data that will be submitted to the RCEB Senior Management Team and the Department of Developmental Services. SPECIFIC DUTIES: • Facilitate the planning process and develop the Individual Program Plan (IPP) for assigned individuals served and closely monitor and oversee the implementation of the IPP. CA advocates on behalf of clients with external agencies, maintains accurate case records, and collaborates with multidisciplinary team members to ensure holistic, wrap-around care. The CA is directly supervised by the HCS Program Manager, and ultimately responsible to the HCS Sr. Program Director. MAJOR DUTIES AND RESPONSIBILITIES: • Conduct comprehensive assessments to identify clients’ housing, health, and support needs., • Develop and implement individualized service plans outlining goals, timelines, and required resources., • Guide clients through emergency shelter intake, housing applications, and placement processes., • Assist with enrollment in public benefits (e.g., Medi-Cal, CalFresh, SSI/SSP) and community resource/support programs., • Advocate for clients with landlords, shelter providers, government agencies, and healthcare systems., • Monitor client progress, adjust service plans as needed, and ensure continuity of care., • Maintain accurate, timely case notes and documentation in the agency’s database and other required systems (e.g., Alameda County HMIS)., • Facilitate referrals to mental health, substance-use treatment, employment services, and other wrap-around supports., • Collaborate with multidisciplinary team members (e.g., outreach workers, mediators, coordinators )., • Participate in regular case conferences and contribute to program evaluation and improvement., • Provide crisis intervention, conflict resolution, and safety planning when necessary., • Conduct community outreach and build partnerships with service providers and local stakeholders., • Perform additional duties as assigned by supervisor to support evolving program and organizational needs. REQUIRED QUALIFICATIONS: • Associate’s degree in social work, psychology, sociology, or a related field and/or 2 years’ equivalent experience., • Minimum of 2 years’ experience in case management, homeless services, or a closely related area working directly with unhoused populations or clients in homelessness prevention programs., • In-depth knowledge of public benefits programs (e.g., Medi-Cal, CalFresh, SSI/SSP) and local housing resources., • Demonstrated use of restorative approaches such as harm reduction, motivational interviewing, theoretical change models, trauma-informed care, non-violent communication, crisis intervention, and emotional regulation., • Demonstrated ability to assess client needs, develop individualized service plans, and monitor progress., • Proficiency with case management databases and systems (e.g., Alameda County HMIS, Salesforce)., • Strong written and verbal communication skills, including documentation and client advocacy with the ability to effectively engage culturally diverse populations and maintain a professional demeanor., • Proficiency with Google Suite, Microsoft Suite, and/or Slack applications, including spreadsheets, word processing, calendars, and messaging tools., • Demonstrated ability to take initiative, develop creative solutions, and execute tasks in an organized and timely manner., • Strong attention to detail, with the ability to multitask, prioritize, and work both independently and collaboratively in a dynamic environment., • Bachelor’s or Master’s degree in social work, psychology, counseling, public health, or a related field and/or 4 years’ equivalent experience., • 3+ years’ experience in case management, homeless services, or a closely related area working directly with unhoused populations or clients in homelessness prevention programs., • Advanced proficiency with Alameda County HMIS, Salesforce, or similar client-management systems., • Demonstrated success in securing permanent housing placements and navigating landlord/tenant processes., • Fluency in additional languages (e.g., Mandarin, Vietnamese, Spanish)., • Established network with local housing providers, emergency shelters, and public agencies., • Ability to communicate, see, and hear effectively., • Ability to use a computer, smartphone, and/or tablet (e.g. iPad)., • Ability to work and walk outdoors for extended periods of time, perform responsibilities in various environments multiple times a day, and travel locally.
