SUMMARY The Diabetic Retinal Screening Coordinator is a dedicated, organization-wide role responsible for the end-to-end ownership of diabetic retinal screening for all eligible patients across all clinic sites. This position serves as the single point of accountability for planning, outreach, scheduling, screening execution, image transmission, results tracking, follow-up, and performance improvement related to diabetic retinal exams. The Coordinator works collaboratively with all clinic staff to design and execute a standardized, high-reliability retinal screening program that improves access, closes care gaps, and supports clinical quality metrics. ESSENTIAL DUTIES Program Ownership • Serve as the organizational lead for diabetic retinal screening across all sites., • Develop, implement, and continuously refine a systemwide retinal screening plan of action, including: Annual screening targets, Outreach strategies, Site-based workflows, Staffing and equipment utilization models, • Partner with the Leadership to ensure alignment with evidence-based guidelines and organizational quality goals., • Standardize retinal screening workflows across sites to ensure consistency, efficiency, and compliance., • Generate and manage retinal screening outreach lists for all diabetic patients who are overdue or due for screening., • Conduct direct patient outreach, including:Phone calls, Text messages, Letters or portal messages, • Coordinate scheduling of retinal screening appointments across multiple clinic sites., • Track outreach attempts, patient responses, and appointment outcomes. Clinical Screening & Results Management • Perform diabetic retinal screenings using retinal imaging equipment in accordance with training, protocols, and scope of practice., • Ensure proper image quality, documentation, and upload to the designated ophthalmology reading platform (e.g., EyePACS or equivalent)., • Maintain accurate and timely documentation in the electronic health record (EHR), including date of screening, image submission status, results receipt, provider assignment for review, and completion of the encounter progress note using the correct EHR template to ensure proper coding, billing, and clinical closure once results are attached to diagnostic imaging (DI)., • Track ophthalmology reads and ensure results are received, documented, routed to the appropriate provider, • Coordinate and track follow-up actions, including: Referrals to ophthalmology, Patient notification of results, Rescheduling of inadequate or unreadable exams, • Close the loop on abnormal findings to ensure patients receive appropriate next-step care. Quality Improvement & Performance Monitoring • Identify barriers to screening completion and propose improvement initiatives to clinical and operational leadership., • Collaborate with the Quality Improvement team to support HEDIS, UDS, and other applicable quality measures., • Provide regular reports and insights to leadership on retinal screening performance and trends. Cross-Site Support & Training • Travel between clinic sites as needed to support screening activities., • Serve as a subject matter expert for diabetic retinal screening workflows., • Provide guidance and informal training to clinic staff regarding: Patient identification, Referral workflows, Scheduling best practices, • Support onboarding and optimization of new retinal equipment or processes., • Ensure proper use, care, and basic troubleshooting of retinal imaging equipment., • Coordinate with leadership on equipment needs, maintenance, and replacement planning., • Maintain compliance with HIPAA, OSHA, and clinic policies related to patient information and clinical care., • Participate in interdisciplinary meetings related to diabetes care and population health., • Assist with development of policies, workflows, and SOPs related to retinal screening., • Perform other duties as assigned that directly support diabetic retinal screening operations. POSITION REQUIREMENTS To perform effectively in this position, the incumbent must have: • Medical Assistant training and certificate of completion/Medical Assistant experience, • CPR/BLS certification, • Working knowledge of Clinic policies and procedures, • Basic clerical skills, including the use of a personal computer, • Must have reliable transportation and ability to travel between different administrative and clinical sites, • Familiarity with HIPPA and OSHA regulations and an ability to maintain absolute discretion over highly sensitive patient information., • Very effective people skills including oral communication skills., • Ability to work effectively with people of diverse cultures, ages and economic backgrounds, • Bilingual skills in English/Spanish – (Preferred) Regulatory Requirements: • Successful completion and acceptable results of post offer physical examination, drug screening, and criminal background check., • Recommended Healthcare Personnel Requirements for vaccinations and preventative testing: Hep B, Influenza, MMR, TDAP, TB Typically, these skills are the result of completion of a combination of appropriate medical assistant training and a minimum of two years of experience in front and back office clinic procedures.
