Job Description General Description: The Wellness Project Coordinator is responsible for the administration of grants focused on chronic disease management and wellness, including cardiometabolic and lung disease. They are expected to collaborates with interdisciplinary teams to lead and implement chronic disease management programs and clinics. The Wellness Project Coordinator manages the day to day activities of the Wellness Center which includes a range of evidence-based, physical activity, and nutrition programs. This is a dynamic position that also includes patient-facing responsibilities, such as outreach and engagement for chronic disease management programs and clinics, screening for social needs, as well as co-leading evidence-based chronic disease self-management programs when needed. Note: This position is grant-funded. Minimum Requirements: • Bachelor’s or Master’s degree (or equivalent experience) in Health Care Management, Public Health, Data Science, Management, Nursing, or related field., • Oral and written fluency in English (fluency in another language a plus, such as Spanish, Arabic, Russian, Vietnamese, or Nepali)., • Experience in design, implementation, and management of grants or grant-funded programs, including reporting, non-personnel budget management, and workplan development., • Experience or prior involvement in the development, improvement, or evaluation of health programs/services in community settings., • Prior training/certification in facilitation of evidence-based wellness programs will be highly prioritized, such as Chronic Disease Self-Management, Diabetes Self-Management, Asthma Home Visiting, Personal/Group Fitness Training, or other related curricula., • Experience in Electronic Medical Record utilization/management. EPIC OCHIN preferred, • Strong theoretical understanding of principles of health equity, social determinants of health, community development, and social justice., • Excellent written and oral communication skills., • Strong professional communication and correspondence skills., • Proficiency in Microsoft Office (Word, Excel, and PowerPoint) and Google Workspace (Email, Calendar, Drive, Docs, Sheets, and Slides) applications., • Ability to develop presentations, deliver presentations, and facilitate meetings with internal teams, stakeholders, and grant funders., • Superior attention to detail and organizational abilities; ability to validate own work as well as team members’ work., • Ability to demonstrate cultural sensitivity with staff, teams, patients, stakeholders, and partners representing diverse cultures, ethnicities, sexual orientations, gender identities, socioeconomic statuses, educational backgrounds, worldviews, literacy levels, and languages., • Ability to listen actively, demonstrate responsiveness to team needs/requests, and integrate feedback thoughtfully and promptly., • Ability to provide constructive feedback, training, and support to community-facing staff., • Coordinate a variety of evolving grants, programs, and research projects focused on chronic disease management and wellness, including diabetes, prediabetes, hypertension, dyslipidemia, cardiovascular disease, stroke, asthma, COPD, and lung cancer., • Grants administration responsibilities will include development and monitoring of workplans for multiple grants, non-personnel budget/fiscal support, data collection and reporting, execution of grant objectives and deliverables, and funder communications/relations., • Exercise a high level of organization and attention to detail to ensure timely tracking, completion, and submission of all grant deliverables., • Maintain positive relationships and responsiveness to grant project officers and technical assistance providers, and ensure successful completion of all required meetings and communications., • In collaboration with the QI and Wellness Project Manager, provide schedule coordination, conflict resolution, training coordination, technical assistance, and validation/monitoring of work products of Community Health Workers assigned to chronic disease management and wellness grant areas., • In collaboration with the QI and Wellness Project manager, design workflows and strategic approaches to integrate grant-required objectives into clinical service delivery., • Collaborate with Primary Care Providers, Nurses, Medical Assistants, Clinical and Retail Pharmacists, Cardiometabolic Team, and Asthma Team to ensure consistency across program workflows, promotions, and reporting of outcomes across chronic disease management programming, • Facilitate meetings, deliver presentations, and foster partnerships with interdisciplinary internal teams, grant funders, and community partners., • In collaboration with the QI and Wellness Project Manager, manage the health center’s Wellness Center, including coordination of a variety of internal and community-based health education, physical activity, nutrition, and wellness programs. Develop and manage Wellness Center policies, procedures, programs, and schedules., • Develop referral and promotion systems to engage priority patients in Wellness Center and chronic disease management programs, clinics, and events., • Organize the delivery of chronic disease self-management educational workshops, including oversight of facilitator training and certification requirements for Community Health Workers, compliance with licensure requirements, and workshop scheduling and promotion, • Continuously expand knowledge and understanding of community resources and services, public health prevention practices, and evidence-based intervention programs., • Develop and strengthen partnerships and bi-directional referral systems with community-based service providers., • Report to and collaborate actively with departmental leadership; provide thorough communications and updates regarding program operations, successes, and barriers; identify and address inefficiencies and solutions., • Position requires prolonged periods of sitting at a desk and typing on a computer; ability to stand for prolonged periods of time; ability to perform moderate activities such as climbing stairs, bending, stooping, reaching, and lifting up to 20 pounds at times; and potential exposure to adverse outdoor environmental conditions. Salary: $65,000 annually
