Job DescriptionSalary: $46,000+ SUMMARY The Housing Stability Case Manager provides direct, strengths-based support to individuals and families experiencing housing instability, including those residing in shelter, transitional housing, and permanent supportive housing (PSH). This position supports residents in maintaining stability, developing housing plans, accessing community resources, and navigating the coordinated entry and supportive housing systems. The role ensures compliance with grant requirements, agency protocols, and best practices in service delivery across the housing continuum. ESSENTIAL DUTIES & RESPONSIBILITIES Other duties may be assigned. Shelter & Transitional Housing (50%) • Provide one-on-one housing-focused case management to residents at Family Haven shelter and transitional housing programs., • Collaborate with internal teams and community partners to maintain 100% occupancy and support residents in rapidly exiting shelter into permanent housing., • Develop individualized Housing Stability Plans within seven days of entry and update weekly., • Confirm client eligibility via Kent Countys Coordinated Entry System and maintain accurate documentation., • Assist with housing search activities, including transportation, landlord follow-up, and rental listings., • Facilitate access to mainstream benefits and community services (e.g., MDHHS, TANF, VA, SOAR, etc.)., • Ensure basic needs (food, shelter, hygiene) are addressed prior to employment or education referrals., • Support shelter operations by enforcing policies, facilitating meal distribution, and ensuring facility readiness for turnover., • Coordinate child and family engagement activities in the shelter environment., • Collaborate with the Volunteer Coordinator and oversee volunteer-led events or donations., • Maintain accurate documentation in Virtual Case Manager and Homeless Management Information System (HMIS). Permanent Supportive Housing (50%) • Provide trauma-informed, housing-first case management to individuals and families placed in PSH units through HUD-funded programs., • Build trusting relationships with clients to support long-term housing retention, health, and community integration., • Conduct routine home visits or in-person appointments at ICCF offices, individualized needs assessments, and annual recertifications in compliance with PSH and HMIS grant standards., • Coordinate care with mental health providers, medical professionals, employment coaches, and other service providers., • Monitor lease compliance, advocate with landlords, and provide mediation support as needed., • Utilize harm reduction and motivational interviewing strategies to support clients with complex behavioral health or substance use needs., • Document services in HMIS and other agency systems (Virtual Case Manager) with attention to timeliness, accuracy, and confidentiality., • Participate in regular case conferencing and ensure coordination with the ICCF Supportive Housing team and partner agencies., • Submit progress reports and support internal and external audits as required. ADDITIONAL DUTIES • Participate in rotating on-call responsibilities, including after-hours crisis response., • Attend required trainings, supervision, and team meetings., • Other duties as assigned. Qualifications + Experience: • Experience with populations experiencing homelessness, families, behavioral health, or substance use recovery., • Commitment to racial and housing equity and an understanding of systemic barriers to housing., • Working knowledge of trauma-informed care, harm reduction, and housing-first principles., • Ability to work independently while managing multiple priorities in a fast-paced environment. Skills and Competencies: • Strong written, verbal, and interpersonal communication skills., • Ability to engage with diverse populations with empathy, cultural humility, and professionalism., • Collaborative mindset and commitment to team-based service delivery., • Flexibility, optimism, and a problem-solving orientation. Preferred Qualifications: • Experience working with BIPOC and LGBTQ+ youth populations, • Lived experience and/or an understanding of youth homelessness, trauma and/or mental health/substance use concerns and knowledge of de-escalation techniques, • Strong knowledge of trauma-informed care, youth development, and crisis intervention techniques., • Ability to handle high-stress situations with professionalism and composure., • Proficient in Google Chrome and data management software., • Required: Valid Michigan drivers license and acceptable driving record, • Preferred: Bachelors degree in Social Work, Human Services, or related field; or equivalent combination of education and experience, • Required or Provided Upon Hire:, • CPR/First Aid, • SOAR training Workweek Requirements: • 40 hours/week, • Schedule includes:, • Monday: 8:30am 5:30pm, • TuesdayFriday: 10am 7pm, • Weekend availability, once a month and/or when needed (Rotating Saturday or Sunday availability 11AM-3PM?), • Participation in on-call weekend and emergency response rotation Physical Demands: Must be able to perform essential duties which include walking, standing, sitting, lifting up to 25 lbs, and conducting home visits. Workweek Environment and Physical Demands: • Combination of office work, field-based outreach, and virtual communication., • May require local travel to housing sites, hospitals, shelters, or partner agencies., • Occasional lifting of files or supplies up to 25 pounds., • Standard MondayFriday schedule with occasional evening or weekend availability.
