JOB PURPOSE • To promote and maintain a high standard of care to all residents, • To assist in identifying and addressing needs of the residents, • Assist in the personal care and grooming of residents, • To support staff with daily activities with the residents, • To act as a mentor to all care assistants, • To support management and to include taking temporary charge in their absence, • To administer medication and the ordering of medication, • To help and advice your team with their key resident’s care plans using the Nourish App. 3. PERSON SPECIFICATION – ESSENTIAL CRITERIA • Must possess an apprenticeship in Health and Social Care level 3, • Must have good leadership skills, • Self and team motivated, • Organised and flexible, • Good communicator, • Active team player but also able to work on own initiative 4. KEY RESPONSIBILITIES • To promote a high standard of direct care to the residents based on their assessed need, • To promote holistic care amongst your team., • To be responsible in team building, motivating staff in their daily activities within the house, general house cleanliness., • To continuously evaluate the quality of care given and regularly assess the needs of the residents in consultation with the residents, their relatives and care team and to effect change required to achieve planned goals., • To work closely with members of the multi-disciplinary team such as district nurse, doctors, social workers, psychiatrist etc., • To administer medication using E - Mar and to also order medication when required., • Liaise with GP, chiropodist and optician as required., • Ensure regular oral screening is undertaken and necessary action taken., • To ensure daily records are made in the resident’s files on the Nourish App., • Care planning and implementation of care plans. It is essential that care plans are up to date, and information is accurately recorded. You will see an alert on the Nourish App when your review is due., • To keep in close contact with the GP surgery via email, regarding any concerns/requests that you may have regarding a resident., • To keep next of kin informed of significant changes in a resident’s condition, always., • To ensure religious, cultural, and emotional support for residents and their families, • To maintain good communication by giving/receiving reports or feedback on residents at the beginning/end of each shift., • Attend and participate in all relevant staff meetings., • To follow agreed standards to ensure that the home runs efficiently., • To act as a mentor and role model to your colleagues and team members., • Provide care assistants with regular supervisions in accordance with the National Care Standards regulations, • To ensure the effective and efficient use of all home’s resources, • Be aware of the fire precautions and fire drill procedure. Take charge in the event of an incident should occur. Understand other procedures in the safe running of the house, burglar alarm, locking gates etc., • To ensure that Data Protection protocols are followed always, only providing confidential information to the named next of kin if the resident lacks capacity or has authorised you to do so.
