Driver/PCAs must want to make a difference in seniors’ lives by working with the interdisciplinary care team to implement and carry out services and activities to proactively meet the needs of our participants. Duties/Responsibilities: * Drives Participants to and from scheduled doctor’s appointments and to the Buckeye PACE Center. * May run errands for the facility, including the hardware store, grocery store, pharmacy, and banks. * Effectively communicates with Participants, families, staff, and other agencies and departments. * Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures that wheelchair lifts and tie-down equipment are properly operated and that Participants are delivered to appointments promptly. * Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes. * Assists passengers on and off the van safely and courteously. Deals with medical equipment and other emergencies that may arise. * May be called upon to support the Participants as a Personal Care Aide (In-Home or In-Center), including showering, bathing, toileting, etc. as needed. * Inspects van and maintains it in a clean, orderly, and safe condition. Reports on the need for vehicle maintenance. * Attends staff and training meetings. * Communicates any change in Participant condition to team members. * Completes documentation as required. * Participate in on-going Quality Initiative training as directed. * Other duties as assigned. Required Skills/Abilities: * Requires valid State driver’s license with a clean driving record. * Must possess the ability to make independent decisions when circumstances warrant such action. * Must work effectively in a team environment and operate handicap-accessible vehicles. * Must be able to deal tactfully and patiently with personnel, patients, visitors, and the public. * Must be knowledgeable of vehicle maintenance policies and procedures. * Must be able to minimize supplies waste, equipment misuse, etc. * Ability to organize work and follow through on assigned tasks. * Must have the cognitive ability to process and perform tasks for Participant safety. * Strong verbal communication skills and can follow written and/or oral directions. Education and Experience: * High School Diploma or GED plus one year of experience working with the frail/elderly population. * Attainment of Q-Straint Academy Certification (Training Provided). * One year of experience working with the frail and elderly population or willing to complete Company paid training to meet this requirement. * CPR and First Aid Certification (Training Provided) Requirements: * Requires valid State driver’s license with a clean driving record. * Successful completion of background check, physical, drug screening, TB Screening Physical Requirements: * Must be able to move intermittently throughout the workday. * Sits, stands, bends, lifts, and moves intermittently during working hours.
