Job Description Occupational Therapist (OT) At Country Meadows, occupational therapy is about helping residents adapt, engage, and live safely with independence. The Occupational Therapist (OT) evaluates, diagnoses, and treats functional limitations affecting daily living, cognition, and safety, delivering resident-centered occupational therapy services that promote participation and quality of life. The OT works collaboratively with residents, families, caregivers, and the interdisciplinary care team, and provides clinical supervision and oversight of Occupational Therapy Assistants (OTAs) as applicable. Services are delivered in accordance with Pennsylvania licensure requirements, professional standards, and organizational policies, with a strong focus on dignity, safety, and individualized care. • Current opportunities: Clinical Evaluation and Treatment • Screens, examines, and evaluates patients through medical history review, systems review, and application and interpretation of appropriate assessments to establish diagnoses and individualized plans of care., • Develops measurable treatment goals and intervention plans in collaboration with patients, families, caregivers, and the interdisciplinary care team., • Implements occupational therapy treatment plans and adapts programs based on patient needs, progress, tolerance, and clinical judgment., • Provides therapeutic services including, but not limited to:, • Activities of daily living (ADLs) and instrumental activities of daily living (IADLs), • Cognitive and perceptual skills, • Muscle strength, coordination, endurance, and mobility, • Sensory awareness and integration, • Sitting and standing tolerance, • Balance and functional mobility, • Joint protection and work simplification techniques, • Orientation and task sequencing, • Application of physical agent modalities as appropriate, • Provides clinical supervision, direction, and delegation to Occupational Therapy Assistants (OTAs) in accordance with state licensure requirements, organizational policy, and professional standards., • Ensures delegated tasks are appropriate to the OTA’s scope of practice and established plan of care., • Designs, fabricates, fits, applies, and trains patients in the use of adaptive equipment and assistive devices, including orthotics and prosthetics., • Selects therapeutic activities appropriate to the individual’s physical and cognitive abilities, condition, interests, and functional goals., • Instructs patients and caregivers in skills and techniques necessary for participation in functional and meaningful activities., • Collaborates with members of the rehabilitation and interdisciplinary care teams to select and implement appropriate therapeutic interventions., • Attends and actively participates in patient care conferences, staffing meetings, and other related interdisciplinary meetings., • Makes referrals to other disciplines or external agencies to address comprehensive patient needs., • Ensures effective and efficient delivery of rehabilitation services, including adherence to established plans of care, patient schedules, and departmental expectations.
