Job Description Job Role: Assessor IMMEDIATE SUPERVISOR: Owners and/or Management Team DESCRIPTION: This position provides direct services to clients and families. The candidate will prove quality assurance checks i.e. reviewing, editing and updating of client files for all programs, initial assessment, linking and coordinating services to families. Additionally the candidate will participate in regular meetings and weekly supervision and perform the task outline below. TASKS: Complete initial assessment of client/family situations and ongoing monthly reports following the approved format. Participates in the service plan within the initial thirty- day period. 1. Take the majority of referral calls., 2. Request referral information (if from the hospital) and/or fill out a referral statement with pertinent information., 3. Check Medicaid Status on Medicaid and Magellan Websites. (Magellan has a recent address, which is an important piece of information). Note: which type of insurance do they have to determine the expected stay., 4. Maintain frequent contact with referral sources regarding the status of the referral (i.e. if we have room or not). This includes making contact with the Program Supervisor to inform them of the referral, checking availability, and making sure that they believe the potential client is an appropriate referral., 5. Schedule and complete preliminary assessments with potential clients (For Crisis: if they are in the hospital)(For MHSS: Schedule the full assessment with the client in their home at a later time) to determine eligibility., 6. Complete the assessment interview and/or intake process, 7. Complete Initial ISP before the end of the assessment interview with the crisis client. The comprehensive ISP is due within a 72-hour window., 8. Complete Assessment Documents and accompanying progress notes., 10. Email or give all assessment documents to whoever needs them and identify a counselor., 11. Communicate with insurance and Keisha regarding authorization status and provide authorization approval info when approved., 13. Complete Weekly referral report and attend weekly management meetings to present report., 14. Mobile Crisis Response Clinician, provide therapy, crisis de-escalation, develop ISP, treatment direction and lead mobile crisis team., 15. And all other duties assigned by the supervisor or executive member., 16. Opportunities for quarterly bonuses in addition to salary. Position is responsible for overall treatment of clients, to include medical, counseling, educational, vocational, and recreational services. Duties include but are not limited to training, direct staff supervision, client counseling, and advocating for the clients, whenever necessary. This position will manage all licensure and government regulations. Tasks: 1. Assist with the implementation of daily structure with QMHP and Clients, 2. Prepare monthly reports on client treatment progress, 3. Act as contact person/case manager for all client-related complaints, 4. Assist QMHP with providing case management services if needed, 5. Develop treatment team to assist with monitoring client’s presenting problem, 6. Review QMHP documentation and monitoring of client status in daily notes, 7. Provide resources to assist in transporting youth/families to community activities, medical appointments, etc. as needed, 8. Provide role modeling that promotes the mission and goals of the agency, 9. Coordinate, monitor and maintain general supplies, office supplies,, 10. Implement standards of ethics, 11. Interviewing potential staff and Case Managers, 12. Monitoring and evaluating staff's individual work activities, 13. Assist with the coordination of admission and intake of clients into the center, 14. Facilitate various groups and assist with the planning of therapeutic recreational activities, 15. Maintain consistent communication with Probation Officers, Social Workers, families and community and referent, 16. Preparing staff schedules, 17. Coordinate licensing and guidelines for CCC, HMO, Credentialing, and updating information., 18. Coordinate, assist and perform staff and client training, 19. Monitor staff performance, evaluations, and staff development REQUIREMENTS: Skills: Case management Client documentation Oral and written communication Group and individual counseling Program planning and coordination Staff Supervision Treatment plan development Crisis intervention Minimum Qualifications: A Licensed Mental Health Profession, LMH-E, LPC-R and LCSW-R with 2 years of successful work history and experience with children and adolescents, families and adults with mental illness and meets the criterion for services. Educational Master Level in the Human Services field and experience equal families in the field of human services field, social work, education or other allied discipline or an undergraduate degree in a profession related to human development social sciences.
