Position Purpose Extend safe, organized access to behavioral-health and addiction services by preparing visits, completing credential-appropriate screenings and clinical support, closing follow-up loops, and sustaining reliable field and telehealth workflows. Regional setting: Region 7 communities identified in the approved regional plan, with field deployment based on access barriers, partner readiness, and service demand; MIH, mobile, home-, community-, partner-site-, telehealth-, and remote-patient-monitoring-enabled care coordinated with regional EMS, hospital, maternal-health, behavioral-health, and community partners. Role in the Region-Specific GROW Model · Coordinate assigned activity across Region 7's MIH, mobile/screening, maternal/infant-health, and distance-barrier work so residents experience a connected rather than fragmented pathway. · Use approved referral and communication workflows with Sullivan-Parke Health Facilities (SPHF), EMS/MIH, hospitals, maternal-health, behavioral-health/SUD, public-health, and community partners. · Support MIH behavioral health/addiction encounters, screening, technology, medication/lab workflows, referral closure, and outreach. · Maintain safe handoffs among MIH/EMS, Valcore clinicians, SPHF, and community treatment/recovery resources. Shared outcome contribution: resident reach, MIH/RPM enrollment, chronic-care follow-up, mobile service days, screenings and immunizations, first-trimester prenatal access, tele-MFM access, closed-loop referrals, and fewer missed or avoidable acute-care encounters. The role is accountable for its documented work and care-process contribution, not for claiming sole causation of population outcomes. Essential Functions · Prepare patients, records, equipment, consents, medication lists, and care-team information for clinic, mobile, home, community, and telehealth encounters. · Collect and document vitals, histories, approved screening tools, point-of-care tests, specimens, and other data only when permitted by credential, competency, order/protocol, and supervision. · Recognize abnormal findings, withdrawal/overdose concerns, suicide or safety alerts, pregnancy/postpartum concerns, and deterioration; notify the licensed clinician immediately and activate emergency protocols when indicated. · Support medication reconciliation, injection/vaccine/medication workflows, laboratory monitoring, appointment preparation, telehealth connection, and patient education only within assigned scope. · Track orders, referrals, treatment connections, missed visits, laboratory results, and unresolved tasks; conduct approved outreach and document closure or escalation. · Maintain clean, safe, stocked, and deployment-ready field/mobile equipment and supplies; follow infection prevention, chain-of-custody, cold-chain, and waste-handling rules as applicable. Measurement, Documentation & Grant Accountability Maintain source records that permit Valcore and the regional coalition to aggregate, validate, stratify, explain, and report performance. Use official measure specifications and approved definitions; identify missing data, denominator limits, duplication risk, and attribution limits rather than estimating unsupported results. Accountability Area: Encounter readiness Required Evidence/Contribution: MIH/mobile/home/telehealth visits prepared, screenings/tasks completed, and exceptions. Accountability Area: Follow up Reliability Required Evidence/Contribution: Orders/results/referrals tracked, outreach attempts, closure, and escalation. Accountability Area: Access Required Evidence/Contribution: Residents supported, missed visits re-engaged, distance/technology barriers documented. Accountability Area: Safety Required Evidence/Contribution: Abnormal findings and urgent risks recognized, escalated, timed, and resolved under protocol. · Follow the approved GROW grant agreement, current E.1 budget justification, regional work plan, logic model, outcome-measure appendix, Valcore policies, and written grant-change controls. Do not materially alter the funded purpose, FTE allocation, initiative assignment, or deliverable without authorized approval. · Create timely, accurate, auditable source documentation for services, outreach, referrals, training, partner activity, time and effort, travel, deliverables, and role-relevant outcomes. Support CMS/State monitoring, quarterly and annual reporting, audits, corrective actions, and record-retention requirements. · Protect information under HIPAA, 42 CFR Part 2 when substance-use-disorder records are involved, Indiana confidentiality law, and Valcore security policies. Use approved systems and the minimum information necessary; immediately report privacy, security, safety, fraud/waste/abuse, and compliance concerns. · Maintain grant-cost integrity. Do not supplant unsupported existing salary costs, duplicate reimbursement, or charge unallowable activity. Keep grant-funded, billable, nonbillable, administrative, and other-program time and costs appropriately separated as directed. Professional Scope, Supervision & Decision Boundaries · Practice only within current license/certification, education, demonstrated competence, granted privileges, standing orders/protocols, and required physician/APRN/RN oversight. A grant work plan never expands professional scope of practice. · Use evidence-informed assessment, escalation, referral, infection-prevention, medication-safety, emergency-response, and documentation standards appropriate to the setting. · Maintain current BLS and any additional emergency, maternal, behavioral-health, or field-safety training required for assigned duties. · A medical assistant does not independently assess, triage, diagnose, create a nursing care plan, change medication, or provide licensed-nursing services. LPN and RN duties differ and are assigned only within Indiana scope and validated competency. · Immediately declines and escalates any delegation that exceeds credential, competence, supervision, protocol, or safe field conditions. · Works within the approved MIH/EMS operational framework and scene-safety hierarchy; does not assume EMS authority unless separately certified and assigned. Required Qualifications · One of the following credentials appropriate to the position offered: recognized medical-assistant certification/registration, active Indiana LPN license, or active Indiana RN license/multistate privilege. · Current BLS; additional competency validation for point-of-care testing, specimen handling, vaccines/medications, equipment, telehealth, and field procedures before performing them. · Experience in ambulatory, community, home-based, maternal-health, behavioral-health, addiction, or mobile-care settings, with reliable documentation and follow-through. · Ability to travel locally, use mobile technology, maintain privacy outside a clinic, and respond calmly to changing field conditions. Preferred Qualifications · Behavioral-health, addiction, MAT/MOUD, psychiatry, community health, or mobile-care experience. · Training in motivational interviewing, overdose response, trauma-informed care, and suicide-risk escalation. · Experience with EHR task queues, HIE, telehealth, and point-of-care testing. Core Competencies · Reliable clinical workflow · Scope awareness · Observation and escalation · Patient engagement · Task closure · Mobile-equipment readiness Compliance, Ethics & Participant Rights · Provide person-centered, trauma-informed, culturally responsive, accessible, and nondiscriminatory service. Use qualified language assistance and reasonable accommodations when needed and follow informed-consent and patient-rights requirements. · Complete required screening, credentialing, exclusion checks, training, competency validation, and annual refreshers. Cooperate with quality-improvement reviews and implement corrective action within assigned authority. · Follow current applicable federal and Indiana requirements, including professional licensing and delegation rules, Indiana telehealth standards, MIH/EMS requirements when applicable, HIPAA, 42 CFR Part 2, emergency and mandatory-reporting duties, and organizational clinical governance. · Immediately disclose and manage conflicts of interest. Do not accept impermissible inducements, falsify documentation, conceal errors, retaliate, or knowingly submit inaccurate program, financial, time, clinical, or performance information. Work Environment, Travel & Schedule · Frequent regional travel and work in homes, EMS/MIH operations, community/partner sites, mobile settings, and telehealth/RPM; schedules may include launch, outreach, or service-event coverage. · May encounter rural connectivity limits, variable field environments, chronic/maternal deterioration, behavioral-health crises, weather, and active emergency-response interfaces; follows field and scene-safety direction. · Reasonable accommodation is available. Final physical and schedule requirements will reflect the actual credentialed duties and deployment model. Role Flexibility & Change Control Controlled flexibility. This grant-funded role is designed to evolve as community needs, CMS/Indiana guidance, approved performance measures, payer requirements, technology, partner capacity, and implementation evidence change. The employee may be assigned related duties, locations, populations, workflows, or approved initiative activities that are reasonably consistent with the role's purpose, qualifications, professional scope, and approved GROW plan. Material changes to clinical scope, credential requirements, grant initiative, funded FTE, budget classification, or committed outcomes require documented leadership and grant-authority review before implementation. Document Control Effective/review date: August 24, 2026; review at least annually and whenever the approved E.1, work plan, grant conditions, role scope, law, or operating model changes. Controlling references: Region 7 approved E.1 budget justification; Region 7 work plan, logic model, and project narrative; GROW RFF and appendices; applicable CMS/HHS post-award terms; current federal and Indiana law and Valcore policy. Document purpose: This description defines current essential functions and grant alignment. It does not create a contract, guarantee funding or employment duration, expand professional scope, or replace current law, privilege, supervision, or clinical policy. Acknowledgment I acknowledge receipt of this job description and understand that duties must be performed within applicable law, credential, competence, policy, supervision, and approved grant scope.
