HonorVet Technologies is seeking a travel nurse RN Case Management for a travel nursing job in Torrance, California. Job Description & Requirements • Specialty: Case Management, • Discipline: RN, • Start Date: 12/01/2025, • Duration: 13 weeks, • 40 hours per week, • Shift: 8 hours, days, • Employment Type: Travel Unit Description – Case Management Department Unit Type: • Caseload: up to 25 patients per day, • 8-hour shifts: 08:00 AM – 04:30 PM, • Additional ED shift option: 03:00 PM – 11:30 PM (flexibility required to meet unit needs), • Weekend requirement: every other weekend, must be available for holidays, • Floating required across service lines, • Conduct full patient assessments, determine level of care, and coordinate treatment plans, • Lead discharge planning including SNF, DME, home health, ambulance transport, • Perform InterQual-based admission reviews for ED, Mother-Baby, NICU (competency highly preferred), • Evaluate and coordinate care for complex populations:, • Adoption/foster cases, • Substance use disorders, • Homelessness, • Behavioral health involvement, • Advanced directives, POLST, protective services, • Support Medicare & Medi-Cal–related workflows, • Independently manage high-volume discharge planning, • Organize and participate in family meetings, • Collaborate with HMOs, IPAs, Medicare, Medi-Cal, and payor systems, • Minimum 2 years acute-care hospital Case Management experience, • MUST include care coordination, discharge planning, and admission reviews, • Acute behavioral health–only backgrounds are not accepted, • Strong knowledge of Medicare guidelines and payor processes, • High proficiency with:, • InterQual (preferred), • Milliman, • Admission & continued-stay reviews, • Benefits verification and authorization workflows, • Admission criteria evaluation, • Concurrent/continued-stay reviews, • Pre-cert and prior authorization, • Medical necessity determination, • Appeals, audits, retrospective reviews, • Discharge planning for diverse clinical populations, • Needs assessment and care planning, • DME ordering, • Case conferencing & family meetings, • Knowledge of regulatory frameworks:, • CMS, DRG, HEDIS, Joint Commission, • ICD-10, CPT, • HIPAA, • OSHA, • Worker’s Compensation, • Strong Microsoft applications proficiency, • Social Services, • Pharmacy, • Physical Therapy, • Respiratory Therapy, • Radiology, • Transport, • IV teams, • Lift teams, • Rapid Response, • Dedicated hubs for:, • Discharge planning (SNF search, home health), • Travel experience required; first-time travelers may be screened, • Orientation: Up to 1 week depending on experience (~40 hours) Founded in 2015, HonorVet Technologies stands as a Service-Disabled Veteran-Owned Small Business (SDVOSB), proudly certified by NaVOBA and the U.S. Department of Veterans Affairs.
