Job Description Early Intervention Services is a HRSA funded program providing medical care and support for people living with HIV. The EIS Case Manager will work to improve, access to and retention in, needed core medical and support services for people living with HIV. The EIS Case Manager will also support the JPAFHC clinic system in providing client-centered services, HIV testing and counseling, care coordination, linkage to services and other needed social support services. The goal of the EIS Case Manager is to ensure timely and coordinated access to medically appropriate levels of care and support services for the patients we serve. The EIS Case Manager role supports all sites and required to commute to any of our sites as needed. For patients at JPAFHC accessing care for HIV, the EIS Case Manager is responsible for supporting continuity of care through ongoing assessment of the clients, and other key family members’ needs and personal support systems to ensure linkage to appropriate services that helps patients becoming and staying HIV healthy. Manhattan Justice is a centralized, court-based screening and referral hub that provides judges, prosecutors and defense attorneys at New York County Criminal Court and Supreme Court with a wide array of social service-based sentencing options in misdemeanor and felony cases. Manhattan Justice is seeking a Case Manager to work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to Senior Social Worker, the Case Manager will be part of a team that provides case management services to the ATI Court. The Case Manager will be responsible for providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. An essential and dynamic feature of the Case Manager’s role will be close collaboration with key stakeholders. They will work regularly with the judge and court staff from the New York County Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Primary Duties and Responsibilities • May perform the duties of a Monitor including resident intakes, check-ins, breath analysis and drug screens, to provide backup when necessary., • In own handwriting, prepares written personalized program for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals., • Reviews plan with residents on a weekly basis and document progress and/or revisions, so resident is gaining the life skills necessary to make a successful re-entry into the community., • Assists residents in establishing personal budget, locating suitable housing, enrolling in educational and vocational programs, and participating in family and community activities, as appropriate, to ensure that they are working towards financial independence., • Monitors facility sanitation and safety. Assigns daily housekeeping chores to residents to comply with facility standards., • May orient residents upon arrival at facility to provide smooth assimilation into program (e.g. have resident complete paperwork, take on facility tour, introduce to other residents, etc.)., • Makes entries in own handwriting into confidential client (resident) files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensures files are secure to protect client confidentiality., • Assists Employment Placement Specialist to verify residents' jobs to ensure residents' employment is suitable under established program plan. Counsel unemployed residents by providing guidance in obtaining employment in cooperation with the Employment Placement Specialist., • Verifies residents' pass plan through pass sponsorship according to contract, so that the resident is accountable for his/her whereabouts in the community., • Prepares terminal report within established time parameters upon resident release to summarize program performance and to satisfy contract requirements., • Participates in staff meetings by sharing observations and recommendations regarding the facility, residents, and staff relations to identify and solve problems and function as a unified team., • Maintains confidential information obtained through job duties regarding employees, vendors, residents, outside agencies, etc., so sensitive information is on a "need to know" basis., • Complies with GEO policies and work rules as described in the company Employee Handbook, and the Facility Policies and Procedures Manual, including directives of GEO managers and supervisors in a cooperative spirit to ensure conformance to company standards., • Adheres to GEO’s attendance policy; be available for all shifts; work overtime as required to ensure adequate coverage of the program and to ensure the safety and security of the residents., • Complies with facility safety rules; take appropriate corrective action to ensure work is performed in a safe manner and without injury to self or others., • Communicates effectively with all levels of staff in a manner that promotes a team spirit and a sense of cooperation., • Performs other duties as assigned. Qualifications Minimum Requirements • Bachelor’s Degree in social, human services, criminal justice or behavioral science programs from an accredited college or university., • One (1) year of experience in human services, corrections or community-based services., • Work experience may be substituted for academic studies exchanging one (1) year of work experience in a related field for one (1) year of academic education., • Total work experience needed in lieu of the combination of education and work experience is five (5) years., • Familiar with community corrections systems and current community corrections practices., • Familiar with community work force needs, public and private agencies and employment services., • Ability to write clear concise reports in English using correct grammar., • Ability to add, subtract, multiply, divide, and calculate percentages., • Must be able to react quickly and appropriately in an emergency situation., • Bilingual in English/Spanish preferred, but not mandatory., • Ability to work with computers and the necessary software typically used by the department. Through connection building and collaboration, Parent Advocates help to empower parents in the case planning process and provide options and choices for parents to make decisions for their families. Job Description Summary: The Criminal Court Case Manager, based in the courthouse walk-in reception center, provides comprehensive, client-centered assistance to individuals and families impacted by violence and abuse. Core services provided by the Case Manager include safety assessment and risk management, orientation to court process, information about victims’ rights and role, crisis intervention and supportive counseling, criminal justice and personal advocacy, and referrals. Essential Job Functions: The Case Manager will work with clients who access the Criminal Court reception center and address their immediate needs. Job Description BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, we feed, shelter, teach, and support our neighbors to build a stronger community. Across our 60+ locations, we provide programs in a variety of service areas, including workforce development, children, youth, families, seniors and homeless prevention. The Jerome Avenue Men’s Shelter (JAMS) provides beds to 200 men with mental illness who are experiencing homelessness. The site offers an array of social services, such as housing case management (including assistance with benefits and gathering vital documents), programming to address mental health and substance abuse issues, meals, and activities. The site has an on-site clinic that can provide primary care and psychiatric services. Residents can remain at the shelter until placed into permanent housing, ideally within nine months of receiving services. RESPONSIBILITIES • Provide direct services to clients, including completing psychosocial assessments, service plan development, referrals, advocacy, assist with entitlements and benefits, Housing, and follow-up., • Manage a case load of 25 clients · Submit HRA 2010Es and/or HHA applications for clients on caseload., • Monitor and document client progress toward service plan goals., • Document all interactions with, or on behalf of, clients., • Maintain familiarity with program resources available on- and off-site., • Coordinate client services with other social service, mental health, medical, employment, educational, and other providers., • Conduct regular case conferences with clients and service providers as required., • Complete program reports on a monthly basis, or more frequently as required., • Act as program contact person for inquiries concerning clients., • Report to, and meet with, supervisor on a monthly basis., • Conduct home visits and other field work, as required., • Perform additional administrative duties as assigned., • Assist with medication monitoring oversight after being trained and certified by BronxWorks senior staff to provide medication monitoring services for clients within the facility.
