Assertive Community Treatment (ACT) Wellspring is transforming the lives of people with serious mental illness who have been chronically homeless with a team of professionals dedicated to the most effective treatments. First, the ACT team moves people who have experienced chronic homelessness from the streets to affordable housing and then addresses each individual’s complex needs using a best-practice, person-centered approach to restore skills, rebuild lives and maintain housing. The ACT team wraps each individual with intensive services of a medical team, housing and behavioral health. Wellspring's Crisis Stabilization Program: The goal of Wellspring’s Crisis Stabilization Program is to assist adults who are experiencing an acute psychiatric episode. With our 24/7 care, we help people avoid hospitalization, achieve stabilization, and recover from acute mental health symptoms such as severe depression, mania, or psychosis. Clients receive intensive treatment, close monitoring with high levels of staff support, and medications prescribed by a psychiatrist, if necessary, in a homelike setting. The 16-bed program has two locations – the Samuel B. Todd Center and the David J. Block Center. Both are licensed, accredited crisis stabilization offering the only services of their kind in the Louisville Metro region. The Wellspring Crisis Stabilization Unit (CSU) Targeted Case Manager (TCM): The CSU TCM provides case management to adults with severe and persistent mental illnesses who are homeless and receiving services in the CSU.
