May perform duties of CT Lead or Imaging Coordinator in his/her absence. Meets productivity levels as established by department benchmarking. Assists other areas during downtimes so to maintain productivity. May be required to perform duties of other modalities within the department to which licensers are held by the technologist/sonographer. Complies with other job descriptions of cross-trained modalities also performed within the department. Registered Nurse-SANE Program-Per-Diem Position Highlights: Position: RN SANE Program Location: Evanston Hospital, Emergency Department Hours: must work a minimum of 24 hours of available on-call per six-week schedule. In addition to being accountable for the overall clinical outcomes of his/her assigned center, they will have a portion of their time allocated to direct patient care as a PCP and/or other clinical duties (amount dependent on number of direct reports). Fills in as needed for center clinical leadership needs, including monitoring daily center census as part of joint center accountability for outcomes., • Plays an active role in the management of their center and helps cover for other providers who may be out for various reasons. It is also expected that each Clinical Director will take an active role as needed in recruiting patients for the center and recruiting and interviewing additional providers for the company., • Managing, mentoring and coaching PCPs in his/her assigned center to deliver outstanding clinical outcomes, including sampling other PCP’s daily huddles within their center, • Leadership rounding with the PCPs (reduced involvement of market clinical leader), • Partnering with Center Operations Director/Market General Manager to drive continued improvement of center financial performance, and helping increase center membership, • Performs other duties as assigned and modified at manager’s discretion., • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other applications used in the company, • Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes, • Fluency in English, verbal and written. Job Description The Department of Ophthalmology at our organization, in collaboration with a local VA hospital, is looking for a full-time Ophthalmologist - Retina Specialist. We are seeking candidates who possess a strong interest in academic pursuits. Key Responsibilities • Provide patient care and clinical services., • Engage in clinical teaching for medical students and residents., • Participate in clinical and/or basic research initiatives. Our department, which includes multiple locations, manages over 65,000 patient visits annually. Position Summary: We are seeking a dedicated Patient Care Coordinator who combines clinical/coordination competence with a strong interest in business development and referral partner engagement. This role will coordinate patient care and simultaneously develop relationships with referral sources (hospitals, nursing homes, physician offices, assisted living) to support our growth and service mission. Key Responsibilities: Coordinate patient admissions, transitions and ongoing care plans in collaboration with clinical teams. Serve as a point of contact for patients, families, and internal care teams to ensure seamless service delivery. Establish and maintain professional relationships with referral sources including hospitals, skilled nursing facilities, assisted living communities, physician practices and community organizations. Educate referral sources about hospice services, company philosophy, eligibility criteria. Assist with marketing/promotional initiatives and in-services to referral partners and community contacts. Participate as needed in evening/weekend/after-hours admission calls and on-call coverage. Qualifications: Bachelor's degree preferred; combination of healthcare experience and business-development or sales experience also acceptable. 1-5 years of relevant experience in hospice/home health/skilled nursing facility environment, or clinical coordination + outreach. Excellent verbal and written communication and relationship-building skills. Ability to work independently and with a team Knowledge of hospice care processes, eligibility, and home care environment preferred. Comfort with CRM systems, tracking metrics, and generating growth reports. Valid driver's license and reliable transportation (role includes field visits).
