Exceptional Benefits: • Minimum of 3 Weeks Paid Time Off (PTO), • Company Vehicle Program, • Flexible Work Schedule, • Mentorship Culture, • Medical, Dental, and Vision Insurance, • 401(k) with Employer Match, • Mileage Reimbursement, • Conduct initial evaluations and ongoing OASIS assessments at appropriate time points, • Deliver skilled nursing services requiring specialized expertise according to physician orders, • Initiate diagnostic, preventive, and rehabilitative nursing procedures, • Demonstrate proficiency in phlebotomy, medication administration, and wound care, • Assume primary responsibility for assessing, planning, coordinating, implementing, and evaluating care, • Develop, implement, and revise individualized plans of care based on patient needs, • Make clinical decisions about when to involve other disciplines or escalate concerns, • Plan and coordinate patient discharges from services, • Educate patients and families on home nursing procedures and self-care techniques, • Teach disease prevention, injury management, and health promotion strategies, • Work toward patient and caregiver independence as the ultimate goal, • Provide counseling appropriate to each patient's condition and learning needs, • Collaborate with physicians, therapists, social workers, and other healthcare professionals, • Initiate and participate in client care conferences for complex cases, • Assign and supervise home health aide services, conducting required supervisory visits, • Supervise LPN/LVNs and paraprofessionals providing care, • Communicate significant findings, problems, and changes to physicians and supervisors, • Report unsafe conditions promptly to appropriate parties, • Participate in quality improvement initiatives and in-service programs, • Stay current with best practices and regulatory requirements, • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing, • Current State License as a Registered Nurse, • Minimum 1 year of experience as a licensed RN in a clinical care setting, • Reliable transportation with valid automobile insurance, • Bachelor of Science in Nursing (BSN), • Experience in home health environment, • Strong clinical assessment and critical thinking skills, • Proficiency in OASIS documentation and care planning, • Excellent time management and organizational abilities, • Effective communication and teaching skills, • Ability to work independently with minimal supervision, • Comfort working in diverse home and facility environments, • Strong clinical judgment and decision-making capabilities Location • Office Location: 770 Kenmoor Ave, Suite 100, Grand Rapids, MI 49546, • 8:00 am – 5:00 pm, Monday through Friday, • Weekend/On-Call Rotation Pay Range$75,000—$85,000 USDBackground ScreeningOptimal Care conducts a background screening upon acceptance of a contingent job offer.
