Job Description Job Title Supported Housing Recovery Coach Reports to Supported Housing Supervisor FLSA / WAGE $41,170.00- $43,640.20 Original Date: February 1997 Revised: May 2024 JOB SUMMARY Reports to the Supported Housing Supervisor with responsibility for working with Supported Housing members on psychiatric rehabilitation goals to foster independence and progress toward mental health and/or addiction recovery. ESSENTIAL FUNCTIONS Interact one-on-one and in group settings with residents: • Form relationships with members by talking with members and reading their histories and assessments in the agency's electronic health record. Socializes with members on each shift to build rapport and a trusting, friendly bond., • Know the goals of each member by reading the Service Plans and become familiar with each goal and the rationale for the goal. Address goals with members during the shift. Inform the Supported Housing Services Coordinator of additional goals as you become aware of them., • Create learning opportunities and teachable moments with members to promote further knowledge and independence., • Supervise individual members as they take medication. Discuss the medications, the proper dosage, the time it is to be taken, etc. Educate members about the medications they take and how to take their medication independently., • Assist members with daily skills such as cooking, general housekeeping, hygiene, and other activities of independent living. Coach members to complete activities of independent living through repetition, example, and working side-by-side with them whenever possible., • Encourage members to participate in community activities offered by Park Center and have members sign up on the sign-up sheets., • Ensure the safety of all members by checking on them regularly during shifts. Know where the members are and what they are doing throughout the shift., • Collaborate with Supported Housing Supervisor and Supported Housing Services Coordinator to create weekly schedule of psychiatric rehabilitation groups., • Facilitate daily psychiatric rehabilitation groups in the home, to include curricula such as WRAP and IMR, as well as other educational and skill building topics., • Engage with members individually regarding their unique goals and utilize the Recovery and Life Skills Tool Kit provided at the house to offer individualized education., • Facilitate weekly menu planning group with members, work with members to create a grocery list, and document in a group note., • Ensure that the house is kept neat and clean by assisting members in completing assigned chores daily, providing education as necessary., • Keep linens clean and neatly organized in closets or other designated areas., • Ensure members’ personal food is maintained in designated area, and kept clean, organized, and in containers., • Notify Supported Housing Supervisor of maintenance issues as they arise., • Perform monthly fire drills (as assigned)., • Create operational lists for the home, including weekly grocery/supply list; weekly menu; weekly chore list., • Document daily progress notes on each member in EHR as well as documenting house activities in the house logbook., • Document when medication is given by initialing medication log, noting on house log, and in individual progress note., • Complete the shift change checklist at the end of every shift., • Clock in and out using the electronic system., • Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds., • Participate in monthly supervisions with Supported Housing Supervisor., • Complete Essential Learning courses by deadline., • Keep up with current issues and correspondence by reading emails on each shift., • Assist with housing events and extra activities as applicable., • Maintain good attendance and punctuality. Follow policy for vacation and sick time., • Work on either Thanksgiving or Christmas day., • Follow all Personnel Policies and Procedures., • Follow HIPAA confidentiality guidelines., • Follow emergency procedures (i.e.: resident crisis, injury, or weather)., • Bachelor’s degree in social work, Sociology, Psychology, or related field with applicable experience or, • High School Diploma or GED, with minimum of three years’ experience in a mental health field, • Experience in a residential setting preferred., • Training in WRAP and IMR facilitation preferred., • Current CPRP certification or willingness to pursue certification upon hire. WORKING CONDITIONS The position involves working with members who have behavioral health concerns and who may at times exhibit behavioral issues. Potential exposure to communicable diseases including the common cold, flu, and hepatitis. Position requires availability to work irregular hours, including evenings, overnights, and weekends. PHYSICAL DEMANDS Must be able to sit, walk, stand, twist and bend for brief periods of time. Occasional reaching and or grasping using hand and/or arms. May be required to move or lift up to 20 pounds. May be required to ascend/descend stairs. Regular use of hands requires dexterity in using the telephone, computer keyboard or mouse. Ability to handle disruptions due to requests from members. DIRECT REPORTS None. SCHEDULE Monday through Friday, 6a-2p or Monday through Friday, 1p-9p WORK LOCATION Nashville, TN
