Job DescriptionJob Title: Patient Care Coordinator Job Description The Patient Scheduler is entrusted with the crucial task of scheduling appointments and dispatching transportation to ensure participants reach their scheduled medical appointments seamlessly. Responsibilities • Create and maintain clinic schedule for the Primary Care Provider, Registered Nurses, and Licensed Practical Nurses, including booking exam rooms and nursing care areas., • Maintain the clinic calendar and input return visit requests and semi-annual assessments into the schedule., • Create and manage schedules for internal specialists, such as dentists, hygienists, optometrists, orthopedic shoe specialists, and psychiatrists., • Schedule appointments for participants to see contracted medical specialists based on referrals made by Primary Care Providers., • Coordinate participants' medical appointment schedules with Adult Day Health and other disciplines., • Maintain contact with specialist providers regarding scheduling., • Dispatch drivers to transport participants to and from all medical appointments and adult day health visits., • Manage all transportation requests and coordinate efficient routing to maximize transportation resources., • Create scheduled routes for transportation and make adjustments to accommodate unanticipated changes., • Collaborate with the Transportation Manager and drivers to determine the safest routes., • Utilize transportation software and medical records software to track and schedule medical appointments for participants., • Obtain outside lab work, medical imaging, and external vendor services as needed., • Ensure necessary paperwork is completed for appointments., • Submit transportation requests in Easy Rides for all appointments., • Order clinic supplies and maintain a system of packing slips and invoices to validate the accuracy of bills received and coordinate delivery of supplies., • Send out appointment notices and make reminder calls to participants, caregivers, and escorts about their appointments., • Request records from hospitals, nursing homes, providers, external specialist appointments, and new Release of Information (ROI) forms., • Create packets for Emergency Room, hospital, nursing home, and Adult Family Home admissions., • Cover breaks and lunch for the receptionist as needed., • Sort and distribute mail and physician faxes., • Serve as a backup for the receptionist at all sites., • Operate office systems and equipment, such as personal computers, multi-line telephones, transcribers, copiers, fax machines, record systems, and other related equipment., • Coordinate work with internal and external contacts.Essential Skills, • At least 2 years of experience in scheduling medical appointments and/or dispatching medical transportation., • Proficiency in Microsoft Outlook and knowledge of Microsoft Office and Excel., • Extensive knowledge of streets in King County., • Fluency in English and familiarity with healthcare, medical terminology, care coordination, EPIC, dispatching, customer support, and customer service.Additional Skills & Qualifications, • An Associate's degree is preferred., • Experience with EMR-Epic is preferred., • Knowledge of the Easy Rides Transportation software program is advantageous., • Strong organizational and interpersonal skills, with judgment, discretion, tact, respect for confidentiality, and the ability to manage shifting priorities., • Excellent verbal and written communication skills., • Ability to support and understand agency policies, legal considerations, and professional ethics., • Demonstrated competency in maintaining positive, collaborative, and constructive interpersonal relationships., • Ability to prioritize work assignments and meet productivity and quality standards., • Ability to work with minimal supervision and exercise independent judgment within the scope of responsibility and authority., • Adaptability to changes in work assignments and environment., • Willingness to assume additional responsibilities and learn new procedures., • Effective organizational and customer service skills., • Ability to work well under pressure, meeting deadlines while completing tasks accurately and on time., • Ability to establish systems to promote efficiencies in the workplace.Work Environment This role is office-based within a fast-paced healthcare environment. The work schedule is from 8:30 AM to 5:00 PM, Monday through Friday. Job Type & Location This is a Contract to Hire position based out of Kent, Washington. Pay and Benefits The pay range for this position is $20.00 - $24.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
