Job Description Success Coach (Case Manager) for Targeted Case Management (TCM) Families & Youth Innovations Plus (FYI+) Position: Full -Time; Hourly/Non-Exempt Salary: $17.98 - $21.76 hourly (3 levels based on experience and education) Reports To: Miriam Gutierrez, Target Case Management Program Supervisor Department: Case Management Program Job Summary The Success Coach (Case Manager) for Targeted Case Management (TCM) will provide specialized case management services to individual client needs. They also create programs to benefit clients and educate clients on available community resources. Success coaching ensures that clients receive services in a timely, appropriate, efficient and coordinated fashion. This position requires that in-person support and/or service delivery be provided by the employee in various settings approved by organizational leadership to include, but not limited to: homes, schools, outdoor settings, courtrooms, offices, shared spaces, street outreach locations, etc. These environments are often inclusive of higher levels of noise and other factors that may present a heightened level of stimuli and safety awareness. When providing or participating in leadership-approved on-line activities, being on-camera and actively engaged are also expected and required as part of this work. What You'll Do: • Coordinate services between providers and participants, • Provide case management services and skill-building services to youth in the program, • Provide housing case management services, • Maintain individual case records on participants receiving services, • Provide specialized case management services to individual participant needs, • Educate participants on available community resources and services, • Create programs and services to benefit clients and the community, • Attend community meetings relevant to program and clients, • Administer and facilitate the completion of appropriate intake forms, assessments, intake packet, and service plans, • Conduct field visits, attend meetings and be available to deliver services during traditional and non-traditional hours (when needed) in educational institutions, places of employment, FYI's main office, JPPO offices, detention facilities or any other setting that enables and promotes, the consistent and structured delivery of case management services, • Organize and facilitate life skills in both individual and group settings, • Must pass a CYFD Criminal Records Check, • Bachelor's degree in Social Work, Family and Consumer Sciences, Criminal Justice, Psychology, Sociology, or Criminology from an accredited College or University., • Four (2) years' experience working with at risk youth or children in an educational, behavioral health, or similar setting., • Excellent communication skills, both written and verbal, • Exhibit skills in comprehending, interpreting, and completing delegated tasks efficientlyCompensation & Benefits:, • Competitive salary commensurate with experience, comprehensive benefits package, professional development opportunities., • Benefits Compensation include health, dental, life insurance, short term and long term disability, supplement plans, • 403(b), PTO, and 14 paid holidays

The purpose of Comprehensive Community Support Services (CCSS) is to surround individuals and families with the services and resources necessary to promote recovery, rehabilitation and resiliency. Comprehensive Community Support Workers address goals specifically in the following areas: independent living, learning, working, socializing and recreation. CCSS consists of a variety of interventions based on coaching and addressing barriers that impeded the development of skills necessary for independent functioning in the community. What You'll Do: • Monitor referrals made to community based organizations, medical care, and other services to support the members overall care management plan, • Provide case management and referral services for client caseload, • Assess, plan, implement and evaluate the needs of patients to develop and facilitate an appropriate plan of care and discharge plan, • Advocate for patients and families throughout the continuum of care for procurement of services and optimal health, • Works closely with families to plan, secure, coordinate and monitor services via community and informal, • resources to assist them in reaching optimum, social, psychological, and physical functioning and to reach their identified recovery and resiliency goals., • Conducts Functional/Needs Based Assessments based on needs or challenges of the family, and developing service plans for goals to be addressed in the course of Case Management Services, • Will facilitate treatment team meetings and updated treatment plans in coordination with client and family in addition to informal and formal treatment team personnel/resources for assigned clients in keeping with agency policy, licensing standards, or as clinically indicated.
