Responsibilities * Manage patient care plans and coordinate services for optimal outcomes. * Conduct assessments to identify patient needs and develop care strategies. * Collaborate with healthcare teams to ensure comprehensive patient support. * Participate in ongoing professional development and training opportunities. * Implement strategies to achieve performance targets and goals. * Monitor patient progress and adjust care plans as necessary. * Educate patients and families on care plans and health management. * Document patient interactions and maintain accurate records. * Utilize data to track performance metrics and improve service delivery. * Engage in regular communication with patients to ensure satisfaction. * Provide support and guidance to junior staff and interns. * Stay updated on industry trends and best practices in case management.
