Job DescriptionDescription: Summary The Lead Registered Nurse Trainer at Envision Unlimited is responsible for supervising and directing staff working in the Community Integrated Living Arrangements (CILA) program. This position assesses and provides for the medical, health, and safety needs of the members in the assigned community living program(s) and provides training and authorization for non-licensed staff. The Lead Registered Nurse Trainer plays a vital role in delivering quality health services, care, and support to individuals with Intellectual/Developmental Disabilities. Essential Responsibilities Supervise and direct nursing office staff in CILA locations. Perform onsite member assessments and training as required. Complete quarterly and annual nursing packets and assessments. Enter HRST assessments and SAMAs onto the Health Risk Screening Tool web form, on initial intake, annually and update quarterly as indicated. • Document specific member medical needs and interventions to include member progress notes, entering data into the Quantum data base as needed., • Monitor lab work and request doctor’s orders for labs as needed. Monitor all aspects of member medication regimes; review medication records; orders and bubble packs; review Medication Administration Records (MARs) at least weekly for all assigned members., • Conduct monthly CILA Site reviews including medication and health/safety quality assurance reviews for all assigned homes, QA cycle medications/MARs, returned bubble packs, MARs, and document monthly. Complete monthly Medication Error Reports by the 15th day of the following month., • Assist in staff utilization of E-MAR system., • Develop interventions (Plan of Care) designed to allow staff to meet the specific needs of the members., • Educate the staff in accordance with DHS Regulations. Develop and implement member-specific training and interventions using best practices., • Communicate with House Managers, QIDPs, Administrators, Associate Division Director, Vice President of CLS, HBS, In-Home Respite Services, member’s physicians, and other medical personnel regularly., • Participate in member-centered service planning, member staffing, and present nursing notes, as requested., • When on-call respond to all telephone calls and messages within 30 minutes and provide appropriate nursing/medical directions, as needed. Document and notify appropriate staff., • Conduct staff medication training and medication administration skills observations on an ongoing basis, including annual Competency-Based Training and Assessment (CBTA) reviews for staff., • Conduct triage-telephone and workshop. Administer first aid at Community Day Services (CDS) locations., • Assist in the facilitation of the member’s hospitalization via admission, discharge/after-care planning with visits to the hospital as requested/needed. Includes nurse-to-nurse communication prior to any hospital discharge and participation in care planning meetings for all members., • Administer insulin as assigned. Other Responsibilities: • Provide staff development and training in the areas of health and safety, medication administration, and other related medical subjects., • Attend and participate in Agency and other in-service programs., • Participate on Division and/or Agency-wide committees, as assigned. Requirements: QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION, SKILLS, and/or EXPERIENCE • Must have a current license from the State of Illinois as a Registered Nurse., • CPR Certification., • Valid driver's license and access to a reliable vehicle., • Two years of clinical nursing experience, preferably with 1 year working with the developmental disabilities population., • Be able to obtain a DHS Nurse Trainer Certificate within three months of employment.
