Ennoble Care is seeking a full-time Clinical Care Coordinator to join our team in an office-based setting in the Tulsa, Oklahoma area, working closely alongside fellow Care Coordinators and Ennoble Care providers. This role supports our clinical operations through effective care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings. While this position is not patient-facing, it plays a critical role in supporting providers and the broader care team within a family-practice–style model of care. Key Responsibilities • Coordinate care activities using Ennoble Care's electronic medical record (EMR) system, • Support providers and clinical teams through accurate documentation, follow-up tracking, and care coordination tasks, • Collaborate closely with other Clinical Care Coordinators and internal care team members in an office-based environment, • Communicate clinical updates, patient information, and care coordination needs to providers in a timely manner, • Assist with care transitions, referrals, and coordination with external partners as needed, • Maintain accurate, timely documentation in accordance with established workflows and standards, • Follow escalation protocols and support efficient clinical operations, • Legally authorized to work in the United States, • Active, unrestricted clinical certification or licensure, such as:, • Patient Care Technician (PCT), • Certified Nursing Assistant (CNA), • Medical Assistant (MA) / Certified Medical Assistant (CMA), • Licensed Practical Nurse (LPN), • Registered Nurse (RN), • Or comparable credential, • Must have minimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting, • Must have strong working knowledge of healthcare terminology and care coordination principles, • Comfortable communicating throughout the day via Microsoft Teams, email, phone, and internal systems, • Ability to cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence, • Strong organizational, time-management, and attention-to-detail skills, • Proficiency with Microsoft Outlook and Microsoft Office, • Ability to work on-site, full-time, Monday through Friday (8:30 a.m. to 5:00 p.m. CST), • No weekends, • No on-call, • Daytime hours, • Seven (7) company-paid holidays annually Please answer all application questions thoroughly.
